PIPPY, ID# 104955, 8 yrs old, 60 lbs, Large Mixed Breed, Brooklyn ACC, Gray / White Female, Surrender Reason: Stray, brought in by finder Shelter Assessment Rating: Medical Behavior Rating: INTAKE PROFILE - BASIC INFORMATION: Pippy is an approximately 8 year old unaltered female large mixed breed that came in to BACC as a found stray. No known injuries. However, having trouble urinating. Has this dog ever had any medical issues? Yes Medical Notes: Trouble urinating. Finder stated that she noticed blood in the urine when she is finally able to urinate. INTAKE NOTES - DATE OF INTAKE, 10/09/2020: Upon intake, Pippy allowed the counselor to collar, leash, scan for a microchip, pet, take picture, and walk without any issues. Had a loose body throughout the intake and was wagging his tail. MEDICAL EXAM NOTES 13-Oct-2020 DVM Intake Exam Estimated age: 8-10 years Microchip noted on Intake? negative History: stray, finder reported hematuria and pollakiuria Subjective: BARH. No csvd. Observed Behavior - loose, calm, easily approachable, allowed physical exam and medical handling. Evidence of Cruelty seen - no Evidence of Trauma seen - no Objective P = wnl R = wnl BCS = 5/9 EENT: Eyes have nuclear sclerosis ou, ears clean, no nasal or ocular discharge noted Oral Exam: adult dentition with grade 3-4/4 dental disease, no oral lesions PLN: No enlargements noted H/L: NSR, NMA, CRT ( 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: FI, no MGTs, no vulvar d/c MSI: Ambulatory x 4, skin free of parasites, ~1cm SQ mass on cranial R shoulder, dry and dull hair coat with ventral dermatitis (lichenification, collarettes, papules) CNS: Mentation appropriate - no signs of neurologic abnormalities Assessment: Hematuria/pollakiuria r/o UTI vs stones vs neoplasia vs other Dermatitis suspect underlying allergies Mass right cranial shoulder Prognosis: Fair to good Plan: CTM while at BACC Start clavamox 13.75mg/kg PO BID x7d until 10/20, recheck Rec UA, BW, AXR with placement to r/o cause of urinary signs Recommend diet trial with placement Rec FNA of mass with placement SURGERY: Permanent waiver due to age *** TO FOSTER OR ADOPT *** TO FOSTER OR ADOPT A NYC ACC DOG, PLEASE MESSAGE OUR PAGE or EMAIL US at [email protected] for assistance.
