Severely emaciated, Paw Pad lacerations with kennel crate feet from being caged constantly if that is not enough this sweet baby is vomiting, diarrhea and who knows why. Please please want you step up for the sweet baby who has barely even started to live. and the entire 4 months of knowing nothing but neglect and cruelty? INTAKE DATE 08/01/2020 SYRUP 99711 MANHATTAN ACC Estimated to be 4 mos old, Lbs UNALTERED MALE, LARGE MIXED BREED STRAY FROM NY 10456 MEDICAL BEHAVIOR EVALUATION: BEHAVIOR RATING: Hx: Stray puppy came in 8/1 with emaciation and paw pad ulcerations. Due to body weight, a fecal sample was sent out and is pending. Yesterday morning, pt started vomiting or regurgitating food and pink fluid, then had diarrhea. Abd rads were unremarkable but did not appear to be consistent with a foreign body. He was started on IV NaCl, initially at 40 ml/hr, lowered to 30 ml/hr overnight. Pt had more diarrhea yesterday evening and it became bloody. He was parvo tested (negative) and started on famotidine, Unasyn, metronidazole, Cerenia, Proviable. Overnight, pt had a great appetite. He had more diarrhea (FS 6/7) but it was NOT bloody. He did not have any more vomiting/regurgitation. S: ENERGETIC this morning - barking and jumping all over the place. He wagged his tail constantly and allowed all handling. O: BAR-H, BCS 2/9, MMs pink and moist, CRT (2 sec EENT: No discharge OU, AU, nose. Clean teeth - deciduous canines, adult incisors. PLNs: Not significantly enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. No coughing or sneezing. Abd: Soft, no pain on palpation, no masses palpated, not distended M/S/I: Amb x4. No skin lesions noted. UG: Male Neuro: Alert and appropriate, no sign neurological deficiencies A: Hx hematochezia - resolving although continued diarrhea 1. Stopped IV fluids this morning, did not pull IVC. If pt continues to eat and drink all day without vomiting or too much diarrhea, okay to pull IVC this afternoon and move out of med iso into general population. 2. Continue Unasyn today but if pt does not have further vomiting, will switch to Clavamox. Will put orders in now for this. 3. Panacur 25 lb dose PO SID x5 days 1088 Pt has been vomiting or regurgitating pink bloody fluid today. This evening he started having diarrhea that is red and thick, fecal score 6/7. Parvo tests were negative. ASSESSMENT: 1. Regurgitation or vomiting bloody fluid - R/O foreign body vs. severe bacterial/viral enterocolitis vs. diet change (refeeding syndrome) vs. stress vs. toxicity vs. GI parasites vs. other cause 2. Hematemesis - R/O severe bacterial/viral enterocolitis vs. diet change (refeeding syndrome) vs. stress vs. toxicity vs. other cause PLAN: 1. Decrease IV fluids to 30 ml/hr overnight 2. Famotidine 10 mg/ml 0.9 ml IV q24 x3 days 3. Unasyn 30 mg/ml 6 ml IV slowly q8 hours x3 days 4. Metronidazole 125 mg PO BID x5 days 5. Cerenia 10 mg/ml 0.9 ml IV q24 x3 days 6. Proviable 1 capsule PO SID x7 days - ideally 2 hours apart from other oral medications 7. Move to medical isolation unit 8. Feed small amounts at a time 1088 VET 991088 2-Aug-2020 Vet Assistant Observations Medical Assistant: 5:19 PM Two Parvo tests were done for syrup at 8:30a and 5:20p both were negative for Parvo Virus VA 991554 2-Aug-2020 Tech Exam L V T Notes: 5:04 PM As per Dr 1657, sedated with 0.4 ml dexdomitor and 0.4 ml butorphanol IM at 2:53 PM. Reversed with 0.2 ml atipamezole IM at 3:30 PM. Took 2 view abd x-rays and ran CBC/chem and uploaded results to vet documents. 20g IVC placed in right cephalic and put on NaCl at 40 ml/hr. - LVT 1390 LVT 991575 2-Aug-2020 2-Aug-2020 Tech Exam L V T Notes: 10:13 AM sample to idexx will be picked up 8/3 after 730pm Hx: -emaciated -vomited food this AM -loose BM's (parvo test negative) BAR H pink 1 sec eent- scant ocular discharge pln- wnl hl- 120hr reg nm ss fp c and e abd- relaxed ug- mi with 2 down msi emaciated neuro- nsf a) emaciated regurgitated p) special diet EN OPG monitor for further v/regurgitating 3PM Continues to vomit Sedate- dexdormitor- 0.4ml butorphanol- 0.4ml Abd Rads- 2 views Decreased detail because of dog's thin body condition; no obvious foreign body IV catheter- 40ml/hr for 4 hours VET 991657 LVT-E 991241 DVM Intake Estimated age: 4m Microchip noted on Intake? scanned negative History: Police were called when this puppy in poor condition was tied outside.
