2 yr. male black/white small dog, stray, 12 pounds: DVM Intake Exam; Estimated age:5; Microchip noted on Intake? n; History : stray; Subjective:severe eye disease; Observed Behavior -allows most handling. after extensive eye tests began threatening to bite but never followed though; T =; P =60; R =wnl; BCS 5/9; EENT: severe eye disease: OU heavy purulent dc, eye ringed with large area of heavy neo vasc. bupthalmic appearance to eyes; os small satin uptake.; IOP OD average 20 OS average 12; STT OD (5mm OS 16mm; Oral Exam: mod tartar; PLN: No enlargements noted; H/L: NSR, NMA, CRT ( 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: intact male; MSI: Ambulatory x 4, skin free of parasites, few benign masses, healthy hair coat; CNS: Mentation appropriate - no signs of neurologic abnormalities; Rectal: poop stuck on anus, anal area irritated; Assessment: KCS, corneal ulcer OS, cannot rule out glaucoma; Prognosis:good/fair; Plan:tab ou bid, optimmune ou bid; although todays exam does no support glaucoma, the severity of the irritation could be bringing down IOP (if uveitis present).; recc repeat iop in future.; recc opthalmology consult; SURGERY: Temporary waiver due to illness PROFILE: This animal came from:Pet ShopOrigin AddressBasic Information:Jojo is a 12 year old small black and white male dog who was surrendered by his owner due to the fact that she could not afford to care for him. The owner stated that she got Jojo from a pet store in Brooklyn. Previously lived with:Owner How is this dog around strangers?When around strangers Jojo is friendly and outgoing. When playing with adults he is gentle. How is this dog around children?Jojo grew up with a child in the home and when playing with her he plays gentle. How is this dog around other dogs?Jojo has not spent time around other dogs in the home so it is unknown how he will react. How is this dog around cats?Jojo has never spent time in the home with cats so it is unknown how he will react. Resource guarding:Jojo isn't bothered if someone was to touch his food treats or toys. Bite history:Jojo has never bit a person or another animal. Housetrained:YesEnergy level/descriptors:medium Other Notes:Jojo isn't bothered if he is restrained, pushed off the couch or disturbed while he sleeps. Jojo will tolerate being bathed brushed and having his nails trimmed. He does not like the blower dryer due to the fact he is scared. Has this dog ever had any medical issues?YesMedical NotesWhen Jojo arrived at the care center he had discharge in both of his eyes. For a New Family to KnowOwner described Jojo as friendly and affectionate. When at home he will follow his owner around. Jojo has been kept mostly indoors and eats both wet and dry food. Jojo likes to sleep at the foot of the owners bed. When using the bathroom outside Jojo will go on the grass or cement. He will also go on wee-wee pads in the home. When left alone in the home he is well behaved. Jojo has never been left alone in the yard or kept in the crate so it is unknown how he will react. Jojo knows how to sit and get down. Owner stated that Jojo does not walk on a leash. when off the leash he will wander but comes back when called. Behavior Notes:During intake Jojo allowed the counselor to collar him take his picture and check his teeth with no problems.
